Registering for a Clinic

It is very important that you pre-register; we cannot guarantee there will be vaccine available if you attend a clinic without pre-registering. This page will walk you through how to properly register. If you need further assistance, you can fill out this form.

From the COLOR registration home page, click the purple button that says “Get Started”. Then, you will need to:

Enter patient birthdate. This helps determine what vaccines the patient is eligible for, based on age. Enter your own birthdate if you are registering yourself; if you are helping someone else register, such as a spouse or parent, enter their birthdate.

Select vaccines. Select the checkbox for COVID-19, Flu (influenza), or both.

If you select COVID-19: It will ask whether the patient has a health condition or is currently undergoing treatment that makes them immunocompromised. If you select No or I don’t know, It will ask whether the patient received a COVID-19 vaccine dose in the last two months. If the patient did receive a vaccine dose in the past two months, they are not currently eligible to receive another dose.

Select an appointment location. You can scroll down the list to select the location that works best for you. Click the purple button that says “Book Here”.

Select an appointment date and time. It will show the date of the clinic as well as available time slots in hour increments. Click on your desired time slot; this will produce a drop-down menu of more specific times. For example, if you selected 3:00 PM - 4:00 PM, you will then be prompted to select a five-minute time slot within that hour (3:00, 3:05, 3:10, etc.).

Contact and patient information. This information is required by the state immunization registry, and city/state officials use it to understand who is getting vaccinated. This page will ask your relationship to the patient, patient contact information, and patient demographic information. When you click the purple button that says “Continue”, a pop-up box will appear, confirming whether the date of birth, phone number, and email address displayed is correct. PLEASE DOUBLE CHECK — this is important so that you can be contacted if needed.

TIP: Please make sure you enter your name exactly the way it appears on your insurance card, if you have insurance.

Additional information. Optional question on whether the patient has a disability.

General prevaccination checklist. These 15 questions are required by the CDC to determine if there is any reason the patient should not get vaccinated. On the day of the appointment, the nurse will discuss the answers with the patient and check that nothing has changed. Please be honest when answering these questions. If there is a condition listed that you have never heard of, it is likely that you do not have that condition.

Health insurance information. Flu and COVID-19 vaccinations are provided at the clinics at no cost to you; however, insurance information must be collected. If you have more than one insurance plan (such as Medicaid and private insurance), select that option.

TIP: If you have trouble filling out the insurance information, select “No, I do not have health insurance” and bring your card to the clinic so that someone can enter the insurance information for you.

Provide your consent. Selecting the checkbox is an electronic signature. Clicking the purple button that says “Continue” will finalize and confirm your appointment date, time, and location.
